      目的:比较垂盆草鲜用/ 干用救治蛇伤药效差异,评价垂盆草鲜用优效性及可能机制. 方法:首先采用蛇毒PLA2酶体外抑制实验评价垂盆草鲜/ 干提取物活性差异;然后采用蛇毒腹腔注射后口服药物治疗法、蛇毒与药物预混合后腹腔注射法、蛇毒肌肉注射后药物浸泡法,评价垂盆草鲜汁浓缩物、干品水提物、干品醇提物对蛇伤治疗作用. 结果:垂盆草鲜药体外抑制PLA2(磷脂酶A2)活性优于干用各组. 小鼠腹腔注射蛇毒后口服高剂量垂盆草提取物能够降低小鼠死亡率,且作用强于干品水提组和醇提组;蛇毒与药物预混合后腹腔注射.垂盆草鲜汁浓缩物高、低剂量均能显著延长小鼠生存时间、降低小鼠死亡率(P</i> <0.05),并显著优于水提组和醇提组;药物浸泡法救治蛇毒肌肉注射造成的蛇伤,鲜药汁浸泡组对动物生存状态、生存率、肌肉组织保护作用均有显著改善(P<0.05),并优于水提液浸泡组. 结论:垂盆草鲜用具有显著的抗蛇毒作用,且鲜用优于干用,其机制可能与降低蛇毒PLA 酶活性、调节Ca2+浓度保护凝血系统功能有关,其鲜用优效性与垂盆草鲜汁中保留更多生物活性成分有关. 本研究提示垂盆草鲜用在蛇伤救治中更具时效性和优效性,具有重要的临床意义,符合传统中药蛇伤治疗“鲜者尤良”理念,也为其他中药鲜用提供参考.

      Objective: Comparing the effectiveness of Fresh/ Dry Sedum sarmentosum on snakebite, and to evaluate the efficacy superiority of fresh Sedum sarmentosum snakebite treatment and discuss the potential mechanism.Methods: Using snake venom Phospholipase A2 (PLA2) enzyme activity inhibition test to evaluate the efficacy of Sedum sarmentosum fresh/dry extract in vitro; then using three in vivo animal experiments as follows, (1) drug oral administration after venom injected intraperitoneally, (2) the mixture of venom incubated with drug intraperitoneal injection, (3) drug immersion therapy after venom injection into the muscle.Results: The inhibition of PLA2 activity by fresh herb extract in vitro was superior to that of dry herb extract. The results of the oral administration of high-dose extracts also showed that the mortality of the fresh Sedum sarmentosum group was lower than that of the other groups, The survival curve of the in vivo protection experiment showed that the fresh Sedum sarmentosum groups significantly prolonged the survival time and mortality of the mice (P<0.05). The Sedum sarmentosum extract immersion therapy was used to treat snakebite simulated by intramuscular injection of snake venom. The results showed that the fresh herbal juice immersion therapy group had better survival status, higher survival rate and muscle tissue protection effect (P<0.05), meanwhile it has better therapeutic effect than the dry herb water extract immersion group. Conclusion: The fresh Sedum sarmentosum has significant anti-venom effect, and the fresh utilization is better than dry utilization. The potential mechanism may be due to inhibit the PLA2 enzyme, and regulate the Ca2+ concentration to protect the clotting system, which may be on count of Sedum sarmentosum fresh juice may contain more active ingredient with antivenoms vitality. This study suggests that the fresh usage of Sedum sarmentosum have good timeliness and superiority value in the treatment of snake venom injury, which coincides with the proverb “the fresh is the better” in the concept of Traditional Chinese Medicine, and could provide a reference for the investigation of other herbal medicine.
